Services Overview
Geriatric Care Management Services
Professional Geriatric Care Managers (PGCMs) are health and human services specialists who help families care for older relatives, while encouraging as much independence as possible. PGCMs are trained in any of a number of fields related to long-term care, including, but not limited to, nursing, gerontology, social work, or psychology, with a specialized focus on issues related to aging and elder care. The PCGM performs a home assessment and then creates a "care plan" which is a list of recommendations for the best services in the community for the elder. The PGCM acts as a guide and advocate identifying problems and offering solutions.
General Assessment Services
Ovation Health helps families determine what their loved one needs. We initially provide a thorough non-medical in-home/resident care health and special needs assessment, conducted by a RN or MSW professional, which incorporates all the information about patients' health and functional status, living conditions, and social support that are needed to support all the nursing homes or home health agencies' responsibilities.
Elderly patients have more health problems, and monitoring the care given to the elderly is a great challenge. Ovation Health ensures that levels of care are adequate by offering general assessments that evaluates a number of healthcare and environmental issues.
Specialized Medical Assessment Services
Medical Assessments are provided by licensed nurses, physicians and social workers. Your family will receive a fair and unbiased report with realistic
recommendations. Because we have no involvement in actually treating the resident/patient we feel we can provide a truly independent medical assessment. Your family will receive a fair and unbiased report with realistic recommendations, independent of the care facility. Whether we provide initial assessments as a part of a Customized Care Management program, or a health risk assessment to assist in garnering important information about a patient quickly, Ovation Health has the capability to meet the medical assessment needs of our clients. We seamlessly integrate our client's medical and non-medical services so that absolutely nothing falls through the cracks. We develop the right plan for our clients’ care, and educate the family on improving all facets of care for the elderly individual.
Verification Services
Ovation Health makes sure that the facility or home healthcare agency is accurate and truthful in its reporting to your family. Ovation Health ensures that levels of care are adequate by reviewing records, data and making visits. Verification services are provided by special request and at an additional fee by licensed nurses, physicians and social workers.
Advocacy Services
Ovation Health helps families get healthcare facilities and organizations to listen. Our mission is to help an increasing pool of seniors secure their rights to healthcare benefits and reasonable, quality medical care by providing education, resources, support and information. We collaborate with other health-oriented agencies, advocacy groups and healthcare providers to secure the rights of all elders to healthcare benefits and quality medical care.
Our clients’ advocates are available to support patients and families in treatment decisions, crisis interventions, or to help individuals’ access resources available within the hospital system. We serve as a liaison between the patient and the facility, we keep the lines of communication open, and we find solutions quickly.
Visitation Specialists
Visitation Specialists are not necessarily healthcare specialists such as RN’s or Physicians. We will utilize a healthcare specialist if the situation warrants such. Visitation requests may be made by family members, a family or patient representative, or other entity with a vested interest in the well-being of the patient or resident. Ovation Visitation Specialists visit for a given number of visits on specified days to a confined or limited mobility individual.
The Visitation Specialist makes their visits equipped with a digital camera to verify visitation dates and will generate a report that will be delivered to family members and/or parties with a vested interested in the health and well being of the resident and/or patient.
