General Assessment Services

Ovation Health helps families determine what their loved one needs. We initially provide a thorough non-medical in-home/resident care health and special needs assessment, conducted by a RN or MSW professional, which incorporates all the information about patients' health and functional status, living conditions, and social support that are needed to support all the nursing homes or home health agencies' responsibilities. Elderly patients have more health problems, and monitoring the care given to the elderly is a great challenge. Ovation Health ensures that levels of care are adequate by offering general assessments that evaluates a number of healthcare and environmental issues.

Specialized Medical Assessment Services

Medical Assessments are provided by licensed nurses, physicians and social workers. Your family will receive a fair and unbiased report with realistic  recommendations. Because we have no involvement in actually treating the resident/patient we feel we can provide a truly independent medical assessment. Your family will receive a fair and unbiased report with realistic recommendations, independent of the care facility.

Whether we provide initial assessments as a part of a Customized Care Management program, or a health risk assessment to assist in garnering important information about a patient quickly, Ovation Health has the capability to meet the medical assessment needs of our clients. We seamlessly integrate our client's medical and non-medical services so that absolutely nothing falls through the cracks. We develop the right plan for our clients’ care, and educate the family on improving all facets of care for the elderly individual.

Verification Services

Ovation Health makes sure that the facility or home healthcare agency is accurate and truthful in its reporting to your family. Ovation Health ensures that levels of care are adequate by reviewing records, data and making visits. Verification services are provided by special request and at an additional fee by licensed nurses, physicians and social workers.

Senior Healthcare Advocacy

It is undeniable that the financial climate puts increasing pressure on our health care providers, forcing them to spend less and less time with each patient, which may result in lapses of continuity of care. Often the families of seniors are not close by, or are unable to pay the attention that is needed to their care. Often public agencies are simply not as effective as a privately paid advocate focused on you or the needs of your loved one. Each condition brings with it several often confusing challenges;

For example... Do you know how to get a second opinion related to your diagnosis? Do you know what questions to ask to obtain the information you need to make truly informed consent? Are all of your prescription drugs compatible, and are they the right dose for you? Who will coordinate your care when you cannot? Do you have a knowledgeable advisor who will lead you towards the best outcome? Is all your medical information available and in one place? These are just a few issues that an advocate can address. 

Senior Lifestyle Management

We are responsive to the needs of our senior clients. We seek to assist our clients with having a quality of life that is designed to inspire the mind, body and spirit.  We strive to empower our clients, by offering services that integrate a mental, spiritual and physical componenet. We can offer continued education services to families and seniors. We assist with senior relocations, external placements, in home services and nursing home care.

Senior Healthcare Oversight and Geriatric Care Management

Senior Healthcare Oversight and Geriatric Care Management services are some of the mostcritical services that we provide. Speak to anyone that has any contact with the medical industry and they will tell you that oversight to the care that you or your loved onerecieves is critical. Senior healthcare is complicated, filled with red tape and nforseenexpenses. We provide the oversight that is imperative to ensuring that you or your loved one does not fall through the cracks. We can follow up on medical visits, explain services and ensure that the specific needs and wishes of seniors are being met.

 Senior Healthcare Consulting

Our senior care consultants are nurses, social workers, gerontologists, and other professionals with the credentials and xperience in elder care and aging matters. We provide you, your loved one or your family information, solutions and ecommendations for long-term health-care and living arrangements that best suit their clients’ needs, finances, and references. We help ensure our clients will have an on-going, optimum quality of life. You should understand that The people who live longer and stay healthier, are people who feel they have some control and understanding of what is going on in theirlives, they are people who have a sense of confidence about the way they care for themselves and the care they are receiving.

Senior Reminder/Check-In Services

If your loved one is a considerable distance from your home, they may need this service. We provide Check-In services. We can provide daily or weekly calls to your loved one just to say hello – or to ensure that there are no problems or issues. If we are unable to reach your loved one, our staff will visit the home to determine if there is cause for concern. And, if necessary, we will allert your family should an issue be present. This will allow you to sleep easier knowing your loved one is being taken care of by our well trained staff.
