Job Description:

This is an Independent Contractor position, perfect for the educated professional not desiring full-time employment with benefits, but seeking a gratifying autonomous position where their knowledge of the healthcare system and local geriatric services is financially appreciated and fully utilized. As our business grows, the Geriatric Care Manager will have a case load of client's that they will be managing through the continuum of care.

Minimum Job Requirements:

• Registered Nurse or LCSW licensure without restrictions in AL
• Case/Care Management or Home Care experience
• High level of local knowledge about geriatric services available to Seniors
• A passion for caring for the elderly is a must
• The Geriatric Care Manager directs the care plan and interventions designed for specific individuals as put forth by the 
client and their family members. This effort may include assessments, counseling, coordination of needed services, 
assigning and ensuring quality delivery of care-giving within residential settings, and assistance with placement if requested.

Essential requirement for this position are as follows:

• Either a Bachelors’ Degree in Social Work, Gerontology, or related field from an accredited college or university
• Or a Registered Nurse Degree with key experience in Geriatric Care Management 

Desirable requirements for this position are as follows:

• Experience in care management, home care, long-term care or other senior-service related field is preferred.
• A working knowledge of clinical geriatric care issues, family dynamics and interventions, education and training,  
supervision, quality assurance and customer service is desirable.
• Monitoring and implementing approved Plans of Care with external Care Coordination vendors and family members. • Knowledge of Care Coordination, working with geriatric and chronic illness                            
• Knowledge of the Long Term Care industry desirable                                                            
• The ability to work within multi-disciplined teams complicated by family interactions               
• Excellent communication skills
• Attending meetings and communicating with families as needed to inform and update them
• Generalized experience in a care/geriatric setting
• Prior case management experience is very helpful 
• The ability to provide guidance to families in interpreting assessment and Plan of care information    
• A reliable automobile with a good driving record and verifiable automobile insurance
• Standard knowledge of computers and how to use E-mail and fax machines                                                      

Responsibilities:

• Promote the physical, psychological and social well-being of our valued clients
• Converse with clients to promote social interaction and provide emotional support
• Be able to evaluate the appropriateness of living situations for clients
• Respond promptly to and positively to family requests for assistance
• Assist with resident functional evaluations to determine if residents’ current level of life/healthcare is adequate
• Assist clients’ family with developing future life plans for resident
• Coordinate client orientation process if they are placed within a new facility and assist with client move-ins
• Function as liaison for our client, their family and communicate effectively and regularly with assisted living facilities, 
nursing homes and health care providers
• Complete written reports and routine client record reviews and updates as scheduled • Demonstrate integrity, compassion   
and professional business practices
• Maintain positive and effective channels of communication

As an Account Executive, you will:

Call on physicians, hospitals, skilled nursing facilities' management, discharge planners, elder law attorneys, financial planners and case managers. You will work with outside BSW’s, MSW’s, RN’s, clergy and healthcare facilities to elicit referrals for seniors, probe for competitive information of families needing the services that we offer within an assigned territory. We encourage you to work smart.

As an Account Executive, you will also…

• Act proactively to create opportunities for new business with new clients
• Build and maintain client relationships
• Assist Ovation Health with establishing a local presence for yourself and our company
• Maintain target lists
• Prioritize accounts in accordance with the market sales plan
• Gather and organize account-related information and provide input on key customer opportunities, service line extensions 
• Create proprosals & negociate pricing
• Call past customers and cold call new sales leads to generate sales
• Set up workshops and seminars to encourage client growth
• Conducting in-home pre-assessment calls
• Provide brief weekly status report on sales & lead activity
• Provide forecasting input to CEO & COO every month for rolling and annual corporate forecasting. Important Qualifications

• Bachelors Degree
• A minimum of one year healthcare or related industry sales experience
• Knowledge of physician, hospital, skilled nursing facility, case management and discharge planning service needs
• Ability to effectively communicate our services with all levels of the healthcare delivery team
• Strong Microsoft Office skills
• Excellent organizational, interpersonal, presentation and communication skills
• Formal sales training highly preferred
• Ability to travel within assigned territory and to sales meetings
